Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Salary: $90,000 – $95,000 annually (based on experience)

Position Overview

A leading home health organization is seeking an experienced Patient Care Manager (RN) to provide clinical leadership and oversight for a team of healthcare professionals serving patients throughout the Temple area. This role is responsible for ensuring high-quality patient care, supporting clinical staff, maintaining regulatory compliance, and driving positive patient outcomes.

The Patient Care Manager serves as a key leader within the organization, fostering a collaborative environment focused on excellence, safety, and compassionate care.

Key Responsibilities

  • Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements.

  • Coordinate, plan, and monitor patient care services to achieve optimal clinical outcomes.

  • Supervise and support clinical staff, ensuring consistent and appropriate delivery of home health services.

  • Maintain effective communication with patients, caregivers, referral partners, field clinicians, and office personnel.

  • Support quality improvement initiatives and departmental performance goals.

  • Promote professionalism, accountability, and service excellence throughout the organization.

  • Ensure compliance with Medicare guidelines and applicable federal, state, and local regulations.

  • Maintain a safe work environment and reinforce safety protocols.

  • Mentor and develop staff through coaching, education, and ongoing professional development opportunities.

Qualifications

  • Current Registered Nurse (RN) license in the state of Texas.

  • Previous home health experience required.

  • Two or more years of management or supervisory experience preferred.

  • Strong knowledge of Medicare Conditions of Participation and home health regulations.

  • Excellent leadership, communication, organizational, and problem-solving skills.

  • Ability to effectively manage multiple priorities in a fast-paced healthcare environment.
