Why This Role Matters Every day, you'll help older adults maintain their independence, improve their mobility, prevent functional decline, and continue living safely in the communities they call home. Your expertise will directly impact participants' quality of life while supporting a care model that keeps seniors where they want to be: active, engaged, and independent. As part of an interdisciplinary team, you'll collaborate with physicians, nurses, social workers, occupational therapists, and other healthcare professionals to provide comprehensive, participant-centered care. What You'll Do You'll make a meaningful difference by: Evaluating participants' strength, mobility, balance, and functional abilities Developing individualized treatment plans that promote independence and safety Providing therapy services in the PACE Center, participant homes, and community settings Collaborating with the Interdisciplinary Team to create holistic care plans Educating participants and caregivers on mobility, exercise, fall prevention, and adaptive equipment Recommending and evaluating durable medical equipment, orthotics, prosthetics, and assistive devices Supervising and mentoring Physical Therapy Assistants and rehabilitation staff Supporting quality improvement initiatives that enhance participant outcomes Helping seniors achieve their highest level of function and quality of life Required Qualifications Graduate of an accredited Physical Therapy program Current Physical Therapist license Current CPR certification Valid driver's license and reliable transportation Preferred Qualifications Experience working with frail older adults or medically complex populations Experience in an interdisciplinary healthcare environment Strong clinical assessment, treatment planning, and communication skills Knowledge of rehabilitation practices, adaptive equipment, mobility training, and fall prevention Why Therapists Choose Blue Ridge Care At Blue Ridge Care, caring for others begins with caring for our team members.
