Family Practice/Primary Care Nurse Practitioner

Client seeks Nurse Practitioner or Physician Assistant for contract assignment starting November 2-December 18 at their Corporate Health clinic in Huntington, IN. Provider will see ages 2 and up for Primary Care, acute episodic care, physicals, and wellness visits. Hours are Monday 8:00am-4:30pm, Tuesday 9:00am-1:00pm, Wednesday 9:00am-6:00pm, & Friday 7:00am-3:00pm. Full days have 30-minute lunch break; Thursdays are closed. Provider must be Board Certified and must have 2+ years of clinical experience to qualify for this assignment; Indiana DEA is also required. Detailed shift needs are November 2, 3, 4, 6, 9, 10, 13, 16, 17, 18, 20, 23, 24, & 30 and December 1, 2, 4, 7, 8, 9, 11, 14, 15, 16, & 18.
