Clinical Infusion Pharmacist

A Clinical Infusion Pharmacist will report to the Sr. Director of Operations and perform a variety of tasks to ensure the correct medication is dispensed to the appropriate patient, at the appropriate dose, and in the appropriate timeframe. In this role, they will have access to Protected Health Information (PHI) and Electronic Protected Health Information (EPHI). A Clinical Infusion Pharmacist obtains complete and accurate disease state, drug, and laboratory information to ensure the medication dispensed is safe, effective, and appropriate for the defined medical condition. The Clinical Infusion Pharmacist will be expected to establish safe practices for infusion therapy handling, and they collaborate and communicate with multidisciplinary team members involved in providing services to patients. A Clinical Infusion Pharmacist manages and directs pharmacy staff in the production of patient-specific medication orders and prescriptions. A Clinical Infusion Pharmacist maintains current knowledge of the latest clinical treatment guidelines, as well as any laws and statutes applicable to pharmacy practice. A Clinical Infusion Pharmacist is responsible for reviewing medication orders and working with medical providers to ensure the most appropriate medical treatment is provided to the patient. In addition, a Clinical Infusion Pharmacist will also be responsible for: Working with other departments to maximize pharmacy performance Maintain and submit documents to ensure compliance Working with Leaders on various projects and goals as needed Clinical Infusion Pharmacist Job Responsibilities and Essential Functions Conduct activity in compliance with state and federal laws and regulations, including maintaining and submitting required documentation Dispense and distribute medication, supplies, and rental items to patients accurately and efficiently Manage and maintain a patient census specific to them with appropriate clinical management Collaborate and communicate appropriately with all multidisciplinary team members involved in providing services to patients Review new orders for therapeutic appropriateness and collaborate with medical providers as needed Maintain PIC registrations/pharmacist licenses in various states as needed Work with the Pharmacy Manager to implement projects as necessary to support the performance improvement program Maintain knowledge of trends, clinical treatment guidelines, infusion therapy standards, and new technologies in home infusion Collaborate with the Pharmacy Manager, Operations, and IT team members to develop customized assessments, per individual disease state or therapy type, specific to payer requirements, etc. Support the Pharmacy Manager with various department projects and goals, as needed Collaborate with the multidisciplinary team on new patient admissions, including pharmacy counseling, initial assessments, and patient education Clinical Infusion Pharmacist Required Qualifications Hold a B.S. or Pharm.D. degree from an Accredited School of Pharmacy in the United States Be licensed by, and in good-standing with, the Missouri State Board of Pharmacy Computer proficiency Excellent and effective communication skills, both verbally and in writing Soft Skills required: Interpersonal Communication: Consistency in displaying active listening, empathy, and the capacity to forge trust with patients Adaptability: Flexibility to adapt to new company policies, technologies, and workflows Teamwork: Collaborate effectively with colleagues to effectively manage the needs of patients Attention to detail: Ability to follow procedures accurately and consistently, maintaining high standards of care Problem Solving: Identifying barriers and communicating solutions to obstacles to promote efficient and cohesive patient care Clinical Infusion Pharmacist Preferred Qualifications Hold licenses in AR, OK, TX, and NE Home infusion/specialty pharmacy experience Physical Requirements and Screenings The physical demands described here represent those that an employee must meet to perform the essential functions of this job successfully.
