LVN Patient Care Coordinator - Hospice

LVN Patient Care Coordinator (Hospice) Join Our Compassionate Hospice Team! Are you an organized, compassionate healthcare professional who thrives in a collaborative environment? We're looking for a Patient Care Coordinator to help ensure exceptional care for hospice patients and their families. In this vital role, you'll coordinate patient services, support our interdisciplinary care team, and help deliver the highest quality end-of-life care. The hourly range for this position at commencement of employment is expected to be between $32 - $36; however, pay offered may vary depending on multiple individualized factors, including job-related knowledge, licensure, skills, and experience. What You'll Do Coordinate daily hospice patient schedules and services Manage patient referrals, admissions, and intake processes Obtain physician orders and verify insurance benefits/authorizations Support interdisciplinary team meetings and coordinate patient care plans Monitor documentation for quality, accuracy, and regulatory compliance Prepare and submit required hospice quality and HIS data Serve as a resource for clinical staff regarding hospice patient care Collaborate with case managers to ensure continuity of care Assist with staff orientation, education, and performance improvement initiatives Maintain patient confidentiality and uphold hospice standards of excellence Qualifications Current California LVN or RN license preferred Minimum 2 years of hospice or home health patient coordination experience Knowledge of California hospice regulations and Medicare/Medicaid guidelines Current CPR certification Strong communication, organization, and multitasking skills Proficient with computers and electronic medical records What We're Looking For Compassionate, patient-centered approach Excellent organizational and problem-solving abilities Ability to work independently and as part of a multidisciplinary team Professional demeanor with outstanding customer service skills Why Join Assisted?
