Home Health Visits Licensed Practical Nurse Marlborough, MA 01752 | 42.349334872 | -71.543102018 Full‑time Licensed Practical Nurse responsible for home health visits, providing exceptional medical care to adult and geriatric clients in Marlborough, Northborough, Southborough, and surrounding towns. Prior clinical experience as a Licensed Practical Nurse is required. Responsibilities Making home visits to clients living in designated territories. Performing assigned duties, including administration of medication, wound care, treatments, and procedures. Monitoring clients' conditions and reporting changes to Clinical Manager and Client Services Manager. Following up with, executing, and properly documenting doctors' orders. Performing client assessments as necessary. Case management and coordination. Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch‑pad tablet. Qualifications A current license as a Licensed Practical Nurse in the state of Massachusetts. Graduation from an accredited and approved nursing program.
