Job Title: Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: 2 years preferred License: State or Compact license must be in hand at time of submittal Certifications: BLS Must-Have: - Previous charge experience preferred - Experience with charting systems required - Proficient with Epic Description: The Case Manager is responsible for medical necessity review, discharge planning, and utilization review. Identifies and communicates changes in the patient's plan of care to the health care team. Maintains detailed knowledge of community resources and the process for referrals. Anticipates patient needs and ensures timely transitioning of patients to an alternative level of care. Documents in patient records in a consistent and timely manner, including initial screenings and discharge plans. The position is open to first-time travelers. A 100-mile radius rule applies; local candidates will not be considered. Compliance with HIPAA guidelines is essential. Experience in acute hospital settings and understanding admission criteria, appeals and denials, benefits eligibility, and care coordination is preferred. Ability to determine medical necessity per evidence-based guidelines is required.
