Position: Home Health Occupational Therapist (OT)

Location:

Fremont, CA; Castro Valley, CA; Ashland, CA; Cherryland, CA; Hayward, CA; Newark, CA; San Leandro, CA; San Lorenzo, CA; Union City, CA; Oakland, CA; Berkeley, CA; Alameda, CA; Albany, CA; Emeryville, CA; Piedmont, CA

Shift Details:

Type: Day Shift
Schedule: 5x8-Hour (08:00 AM - 04:30 PM)
Guaranteed Hours: 36 hours per week
Patient Visits: Expected to see 5-6 patients per day

Start Date:

ASAP (Compliance may take about 2 weeks)

Job Responsibilities:

Deliver occupational therapy services to patients in their homes.
Manage a caseload of home health patients while adhering to organizational productivity standards.
Maintain timely communication and accurate documentation of patient progress and care.
Utilize provided documentation tools effectively and return them upon completion of assignment.

Requirements:

1 year of preferred home health experience.
Must hold a valid California State Occupational Therapist license.
BLS certification required.
Possess reliable transportation, with occasional travel of up to 50 miles between patient visits.

Additional Information:

Weekend patient visits are optional to help meet productivity goals.
No on-call duties required.
Locals and first-time travelers are welcome to apply.
Any color scrubs are acceptable for attire.
Free parking is available on-site.
