This role involves working closely with patients in their homes, providing essential health assessments, care plan implementation, and case management to ensure comprehensive health maintenance and disease prevention.

Key Responsibilities of the RN - Home Health - Travel

• Observe, assess, plan, implement, and evaluate nursing support for individuals in community or facility settings
• Collaborate with health professionals/providers to address the health needs of clients
• Coordinate communications and actions related to health and safety supports
• Manage medication processes and ensure compliance with policies and guidelines
• Maintain thorough documentation and communication with healthcare teams
• Direct and supervise activities of licensed practical/vocational nurses
• Provide health and safety training for employees and staff

Requirements of the RN - Home Health - Travel

• RN compact state license
• Home Health experience, including HCHB and OASIS
• Proficiency in IV, wound care, PICC line, wound vac, Trach, lab draws
