Job Opportunity

This is a 13-week home health contract assignment for a professional providing therapy in a home health setting. The position is designed for dedicated individuals who will be traveling to patients' homes and attending periodic office meetings.

Schedule

• Shift: Day 5x8-Hour (08:00 - 17:00)
• Hours: Monday - Friday, 8 AM - 5 PM
• Weekend Rotation: None
• On-Call Requirement: None

Responsibilities

• Conduct home health assessments and provide therapy to patients.
• Complete all necessary documentation within 24 hours of patient visits.
• Travel to patients' homes within the assigned territory (Greenwich/Stamford, CT).
• Participate in periodic office meetings.

Travel Requirements

• Candidate should expect to travel approximately 300-400 miles per week.
• Mileage reimbursement will be provided at the IRS standard rate.
• Candidates must be comfortable driving distances of 50-75 miles between patients.

Productivity Expectations

Each candidate is expected to handle an average of 5-10 patients daily, achieving a total of 30 points per week, with each type of visit and activity assigned a different point value.

Qualifications

• Minimum of one full year of adult home health experience is required.
• Previous home health or hospice experience is preferred.
• Must hold a valid driver's license and have reliable transportation.
• Must have agency-required liability insurance.

Certifications

• Current CPR certification.
• BLS certification is required.
