Hourly Rate/Range: $68+ per hour

Qualifications

• 1. Experience: 2-3 years hospital pharmacy experience and/or PGY-1 residency preferred.
• 2. Training/Certification: Pharmacotherapy or other relevant Board Certification preferred or willing to obtain within 1 year of eligibility.
• 3. Licensure: New York State Pharmacist license

JOB SPECIFIC COMPETENCIES

• * 1. Demonstrates sound judgement when providing safe, effective, and compassionate care for patients including solving patient-specific medication related problems, IV and non-sterile preparation, and various aspects of the medication use process.
• 2. Participate in multidisciplinary rounds/conversations and regularly consult with prescribers concerning best treatment options.
• 3. Takes responsibility for patients who require therapeutic monitoring of medications including ordering appropriate labs, adjusting medication doses, and notifying appropriate healthcare providers as needed.
• 4. Documents all required activities in a timely manner.
• 5. Identifies and engages in organizational and department quality improvement activities to improve patient care, medication use process, and pharmacy operation.
• * Document on addendum age specific competency.

• 1. Completes projects/assignments within projected time frames and expectations.
