Position Summary As a Foster Care Case Manager (FCCM), you will provide safety and services to children and families in foster care. Your duties include facilitating Family Support Team meetings, developing individualized care plans, coordinating services across settings, and documenting support activities. You will promote effective use of services to achieve permanency. Essential Job Functions Follow all contractual and agency policies and procedures. Maintain a caseload of 12 to 15 children, adjusting for vacancies or extended absences. Provide full case management services from first assignment to case closure or transfer. Document all activities and maintain data entry in FACES and CSWIS accurately and timely. Maintain at least monthly face‑to‑face contact with each child and family, and regular telephone contact with providers. Attend court hearings and coordinate Family Support Team meetings. Participate in consortium on‑call schedule. Requirements

• Bachelor’s degree in social work, psychology, counseling, education, criminal justice, or a related human services field (master’s preferred).
• Minimum 2 years of relevant case management experience.

Skills Strong verbal and written communication; represent PCHAS professionally. Build and maintain community relationships; manage conflicts with families. Excellent time management, organization, and planning skills. Flexible schedule, available during varied times, including outside normal hours. Maintain confidentiality and HIPAA standards. Proficient with agency‑approved software and data entry systems. Analytical thinking and conflict resolution; culturally competent. Other Requirements

• Valid driver’s license with risk‑free driving record.
• Child abuse/neglect, criminal history, and sexual offender screenings by the Missouri Department of Health and Senior Services and Family Care Safety Registry.
• Fingerprint screening by Missouri State Highway Patrol and FBI.
• Pre‑employment drug screen and physical exam.
• First aid and CPR certification.
