Outpatient Therapist (LPCC/LISW)

Outpatient TherapistThe Outpatient Therapist provides high-quality, evidence-based behavioral health services to individuals, families, and groups in an outpatient setting. The Therapist is responsible for assessment, diagnosis, treatment planning, and delivery of therapeutic services in accordance with Ohio licensure laws, agency policies, and applicable state and federal regulations. Services are delivered using a person-centered, trauma-informed, and recovery-oriented approach.Supervisor: Clinical Supervisor or DesigneeDuties and Responsibilities:Conduct comprehensive behavioral health assessments, including diagnostic evaluations and psychosocial assessments.Develop, implement, and regularly update individualized treatment plans in collaboration with clients and the interdisciplinary team.Provide individual, family, and group psychotherapy consistent with scope of practice and licensure.Utilize evidence-based and best-practice therapeutic modalities appropriate to client needs (e.g., CBT, DBT, MI, trauma-informed care).Maintain accurate, timely, and compliant clinical documentation in the electronic health record in accordance with OMHAS, Medicaid, and agency standards.Participate in interdisciplinary team meetings, case consultations, and care coordination activities.Collaborate with internal and external providers, referral sources, and community partners as needed to support continuity of care.Provide crisis intervention, safety planning, and appropriate referrals for higher levels of care when clinically indicated.Maintain ethical standards, professional boundaries, and client confidentiality in compliance with HIPAA and 42 CFR Part 2, when applicable.Participate in required supervision, training, and continuing education activities.Comply with all agency policies, procedures, and applicable local, state, and federal regulations.Other duties as assigned.The duties and responsibilities above represent the most significant duties of this position, but do not exclude other assignments that would be within the qualification and responsibility levels of this position.RequirementsMinimum Qualifications:Master's degree - counseling or social workKnowledge of evidence-based treatment models and recovery-oriented systems of care Reliable transportationStrong clinical documentation and organizational skills.Ability to work independently and as part of a multidisciplinary teamAbility to meet agency requirements for driving insurabilityAmerican Heart Association (AHA) BLS CertificationPreferred: Experience working with individuals with co-occurring mental health and substance use disorders.Preferred: Familiarity with Medicaid billing and documentation requirements.Preferred: Experience in integrated care, community mental health, or FQHC settings.Licenses or Certifications Required:Unrestricted LPCC/LISW license to practice in the State of OhioUnrestricted State of Ohio driver's licenseCompliance:This position requires compliance with AFS' written standards, including its Compliance Program and Standards of Conduct and policies and procedures.
