Case Manager 3

The Department of Economic Security, Division of Developmental Disabilities seeks a Case Manager 3 to support individuals with developmental disabilities and their families in Flagstaff. The role involves conducting home and site visits, developing person-centered service plans, facilitating meetings, collaborating with community agencies, and maintaining case records. The position requires a valid driver's license and demands knowledge of case management practices, HIPAA compliance, and community resources. Key qualifications: Arizona licensed Registered Nurse OR Bachelor's degree in Social Work, Psychology, Special Education, or Counseling with 1+ year case management experience Minimum 2 years case management experience with elderly, physically disabled, developmentally disabled, or seriously mentally ill individuals Valid driver's license and ability to drive to conduct onsite visits Level One fingerprint clearance card for working with vulnerable adults Knowledge of HIPAA, federal and state statutes, and person-centered service planning #J-18808-Ljbffr Dept of Economic Security
