Physician Assistant

Signify Health seeks a Physician Assistant to perform comprehensive in-home and virtual evaluations for diverse, often vulnerable patient populations. You will obtain detailed histories, conduct focused physical exams, interpret diagnostics, and document findings in our EHR to support value-based care programs. Partnering with care coordinators and physicians, you’ll identify care gaps, optimize chronic disease management, and facilitate appropriate follow-up.
