In-Home Health - NP/PA (Full Time) - Bronx, New York

About the Role Signify Health, part of CVS Health®, is looking for a full‑time Clinician (Nurse Practitioner or Physician Assistant) who will deliver in‑home health evaluations, engaging with patients in the comfort of their own homes and bridging gaps in care. Key Responsibilities Bring heart into every visit and build trusted relationships with patients. Deliver in‑home evaluations lasting about 45 minutes, including medication and medical history review, physical assessment, and any ordered diagnostic tests (e.g., diabetic eye exams, spirometry). Use an iPad and intuitive clinical workflow for each evaluation. Conduct virtual visits as needed. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.). Travel locally and within licensed states as required. Maintain a safe and comfortable environment for patients, accommodating pets, secondhand smoke, or allergens. Required Qualifications Active, unrestricted license(s) in coverage area(s). Board certification (required). Multi‑state licenses (preferred). Evidence of professional experience (NP or PA). Preferred Qualifications Previous experience in home health or preventive care settings. Compensation The typical pay range for this role is: $95,738.00 – $206,206.00. This range represents the base hourly rate or base annual full‑time salary and will be adjusted based on experience, education, geography, and other factors.
