Job Description The Opportunity: Case Manager in a residential setting providing SUD and related therapeutic interventions. As Case Manager, you'll have a dynamic and critical role providing day to day support for the residents in our Recovery Services Residential program ensuring a safe and secure therapeutic environment. As Case Manager you'll run groups and provide individual case management to residents with substance use, mental health, and/ or co-occurring disorders. Case Manager Responsibilities: Facilitate a safe and supportive environment for residents. Perform weekly case management sessions for up to 9 residents. Develop and provide individualized action plans for each resident to help them reach their goals to achieve independence. Help clients identify appropriate providers and programs and assist with eligibility, applications, and appointments. Provide resources and coordinate services with other VOAMASS programs and outside agencies. Create and facilitate groups with residents in the areas of personal responsibility, social skills, and community living skills and behavior. Complete documentation in Care Logic such as progress notes, resident treatment plans, and other paperwork and ensure all documentation meets the requirements of our regulators. Share information with other staff about assigned residents' progress as appropriate. Attend staff meetings (group supervision) and participate in individual supervision. Monitor medications and Medication Log and ensure all medications policies, including distribution and destruction are followed. The Case Manager is about making a difference in the lives of the people we serve. The Case Manager will bring the following qualifications: Bachelor's Degree in related field and a minimum of two years' experience in mental health, substance use treatment, and/or co-occurring disorders. CADC may be substituted in place of a Bachelor's Degree. Demonstrated strong written and verbal communications skills required. Knowledge of milieu therapy in residential settings; substance use modalities, harm reduction approaches, trauma informed care, whole person health approaches, dual diagnosis, and commitment to meeting the needs of the most vulnerable. Computer proficiency in electronic health records and MS Word and Outlook.
