Solutions-Based Family Case Planner (Child Welfare)

HeartShare seeks a Family Case Planner to deliver casework counseling for families with children under 18 at risk of placement. The role uses the Solution Based Casework model to partner with families on concerns and service plans. Responsibilities include direct services, intake and crisis intervention, home visits, and participation in supervision and Family Team Conferences.
