Summary Serves as facilitator for the pharmaceutical care team; participates in ongoing education of team members; coordinates quality improvement initiatives of the team; ensures the safe, efficacious, and cost‑effective use of medications in assigned patient populations. Minimum Qualifications Formal Education Required: Doctor of Pharmacy Licensed or license eligible by the Ohio State Board of Pharmacy Experience & Training Required: Completion of residency training in pharmacy practice or three (3) years of increasingly responsible experience in a pharmaceutical care position that has provided skills in pharmacy education and quality improvement. Prefer PGY2 in geriatrics. Other Skills, Competencies and Qualifications: Ability to communicate verbally and in writing effectively Ability to operate a computer terminal, hand‑held calculator, and other office equipment; manipulate fine instruments; and prepare pharmaceutical entities in minute proportions. Ability to work well within a team environment: accept and offer honest and constructive feedback, support team goals, encourage other team members, collaborate with patient care team members to consolidate work, solve problems, create new methods, and/or reduce costs. Ability to be self‑directed in executing responsibilities within a team structure Working knowledge of Ohio pharmacy law Flexibility/adaptability to meet environmental constraints and demands common with changing reimbursement and managed care targets Ability to interact effectively with populations of patients/customers with an understanding of their needs for self‑respect and dignity Level of Physical Demands: Sedentary: Exerts up to ten pounds of force occasionally and/or a negligible amount of force frequently Hazards: Exposure to blood, blood fluids, and sharps; risk of falls, strains, contagious diseases, cuts from broken glass, needle puncture, etc.; possible exposure to selected hazardous substances (e.g., alcohol, chemotherapeutic agents, etc.) Equal Opportunity Employer / Veterans / Disabled Equal Opportunity Employer/Veterans/Disabled Compensation $60.24/hr – $90.35/hr. The salary range is base salary exclusive of any bonuses or differentials.
