Responsibilities:

• Coordinate the development and implementation of care plans for our residents in collaboration with other healthcare professionals

• Conduct thorough assessments of resident needs and abilities to develop individualized care plans

• Ensure that all necessary documentation is completed accurately and in a timely manner, including MDS assessments and care plans

• Work closely with the interdisciplinary team to identify and address resident needs and concerns

• Provide education and training to nursing staff on MDS assessment tools and care planning principles

• Collaborate with other departments, such as therapy and social work, to ensure comprehensive care

• Participate in quality improvement initiatives to enhance the care we provide
