Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs to facilitate effective return to community activities. The therapist serves as an educator to patients, families, students, and coworkers while upholding professional standards and organizational values.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document medical records clearly, accurately, and in a timely manner.
• Provide care under signed, valid physician orders and within insurance guidelines.
• Participate in student programs as appropriate, ensuring a positive learning environment.
• Develop and actively pursue an individualized professional development plan.
• Adhere to discipline-specific State Practice Act and maintain professional licensure.
• Engage in performance improvement initiatives to enhance service quality.

Schedule

This position operates on a 5x8.0 shift schedule.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience is required.
• Current CPR certification.
• Maintains professional licensure in good standing.
