Field Outreach Case Manager - Homeless Services
HOPICS
HOPICS is seeking a Metro Outreach Case Manager to join a multidisciplinary team and perform professional case management in outreach to homeless individuals. Under supervision, you will assess needs, develop service plans, and coordinate housing and service linkages across Metro platforms. You will work with law enforcement partners and community stakeholders to engage PEH, connect them to housing, mental health services, substance use treatment, medical care, and other supports while #J-18808-Ljbffr HOPICS
