Physical Therapist, PT - Full Time, Per Diem

Position Type: Full-time and PRN

In this rewarding role, you'll provide comprehensive physical therapy services to residents in a vibrant subacute rehabilitation setting. Your expertise will play a crucial role in helping patients regain their independence and improve their quality of life.

Key Responsibilities:

Assessment and Evaluation: Conduct thorough evaluations of patients; physical abilities, limitations, and rehabilitation needs.

• Individualized Treatment Plans: Develop and implement personalized treatment plans to address specific goals and facilitate recovery.

• Therapeutic Interventions: Utilize a variety of therapeutic techniques, exercises, and modalities to enhance mobility, strength, and functional abilities.

• Patient Education: Educate patients and their families on rehabilitation processes, home exercises, and injury prevention strategies.

• Collaboration: Work closely with a multidisciplinary team of healthcare professionals to ensure coordinated and comprehensive patient care.

• Progress Monitoring: Regularly assess patient progress and modify treatment plans as needed to optimize outcomes.

• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, and progress notes.

• Qualifications:

  Education: Doctorate in Physical Therapy (DPT) from an accredited program.

• Licensure: Current state licensure or eligibility for licensure as a Physical Therapist.

• Experience: Previous experience in a rehabilitation setting preferred, but new graduates are encouraged to apply.

• Skills: Strong communication, interpersonal, and organizational skills; ability to work effectively in a team-oriented environment.
