What You’ll Do As a Health & Wellness LPN, you will serve as an important clinical partner within the wellness team. You will help guide patients through their healthcare journey while supporting both medical and wellness-based treatment plans. Patient Care & Clinical Support Partner closely with Dr. Barnett to delivery exceptional patient care Assist in patient assessments and ongoing treatment monitoring Administer medications and treatments within scope of practice Perform injections, blood draws, vital signs, and clinical procedures Monitor patient progress and treatment outcomes Assist with Direct Primary Care visits and wellness consultations Support implementation of individualized treatment plans Wellness & Preventative Care Build strong relationships with patients through ongoing communication Provide encouragement, accountability, and support Answer patient questions and coordinate care needs Help create an exceptional patient experience Documentation & Coordination Maintain accurate clinical documentation Ensure compliance with established protocols and standards Coordinate patient communication and follow-up Assist with care planning and ongoing monitoring What Makes Someone Successful in This Role The ideal candidate is passionate about health, wellness, and helping patients achieve lasting results.
