Registered Nurse (RN) | Hospital at Home

Overview The Registered Nurse (RN) – Hospital at Home provides acute-level nursing care to patients receiving hospital services in their homes. This role delivers comprehensive, evidence-based clinical care consistent with inpatient standards while promoting patient safety, comfort, and recovery in the home setting. The RN functions as part of an interdisciplinary care team and is responsible for assessment, clinical intervention, care coordination, and patient education within the Hospital at Home model. Responsibilities Review physician prescriptions and communicate the nursing care plan to other members of the health care team. Coordinate with other healthcare providers and provide physical, emotional, psychological, and spiritual support to patients, families, and friends. Perform interventional care in the home utilizing clinical expertise and problem solving to make recommendations to physicians based on comprehensive assessments. Develop nursing care plans by obtaining medical histories, examining patients, documenting physical and psychosocial conditions, drawing labs, and providing respiratory care. Teach healthcare requirements by establishing patient care goals, instructing patients and families on medications and self-care skills, and providing information and resources. Evaluate patient care by adhering to therapeutic standards, measuring health outcomes, reviewing records, and modifying nursing care plans as needed. Maintain a safe, secure, and healthy patient environment by following infection-control policies, medication administration procedures, and controlled substance regulations. Document patient care services by charting in patient and department records. Keep patient care equipment operating by following operating instructions and troubleshooting breakdowns; maintain supplies and perform preventive maintenance. Maintain competence in nursing capabilities as outlined by leadership. Qualifications Registered Nurse (RN) – Board of Nursing; active license in the state of practice. 1‑3 years of experience in Med‑Surg, ER, or home health (preferred). Ability to work the specified hours (8 am‑8:30 pm, agreed on weekends and holidays). A willing flexibility to accommodate shift needs. Visual acuity adequate for performing position duties; clear communication skills; ability to hear, understand, and distinguish speech and other sounds.
