Licensed Practical Nurse (Nights)

Hospice Licensed Practical Nurse (Nights) Salary: $31.25-$34.13 BOE Schedule: Mon/Tues/Wed | 8 PM-8 AM Location: Hamilton County, Ohio, and surrounding area Requirements of the Hospice LPN role: * Licensed Practical Nursing Degree * A minimum of 1 year of Clinical experience/Skilled care * Hospice experience preferred * Valid Drivers License and Auto insurance Coverage * Reliable transportation: Willing to travel About the Hospice LPN role: * Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients. * Provide on-call services after hours, and complete scheduled skilled visits. * Assist Registered nurses in carrying out the plan of care. * Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders. * Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed. * Assist in the reinforcement of patient and family education regarding end-of-life care. * Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning. * Keep the RNCM informed of changes noted during the process. * Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading. * Perform other duties as directed Why work with Care 360 Hospice: * Medical, dental, and vision health Insurance * Paid Time Off * 401(k) Matching * Life Insurance * Growth opportunities
