LTSS Field Clinician & Care Coordinator

Elevance Health is seeking an LTSS Service Coordinator – Clinician to coordinate care for members throughout Virginia. This role involves identifying high-risk individuals, assisting in assessments, and participating in care coordination. Candidates must hold an LPN/LVN, LSW, LCSW, LMSW, or equivalent license and have at least 2 years of experience working with individuals facing chronic illnesses. The position allows flexibility with field-based work, including travel to client sites.
