Hospice Chaplain

Gentiva is seeking a Hospice Chaplain to provide spiritual and emotional support to patients and families in their homes. In this role, you will assess spiritual needs, offer counseling, prayer, and ritual support across diverse faiths and beliefs, and participate in interdisciplinary care planning. You will collaborate closely with nurses, social workers, and physicians, document visits, and provide grief and bereavement support.
