Licensed Practical Nurse (LPN) - Shrewsbury Rehabilitation and Nursing at Southgate

Job Summary:

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our team at Shrewsbury Rehabilitation and Nursing at Southgate in Shrewsbury, Massachusetts.

About the Role:

As an LPN at Shrewsbury Rehabilitation and Nursing at Southgate, you will provide direct nursing care, supervise and support nursing assistants, and collaborate with the interdisciplinary care team to ensure excellent resident outcomes. You will also maintain infection control, safety, and regulatory compliance standards, and support a respectful, positive, and team-oriented environment for residents, families, and staff.

Responsibilities:

• Providing direct nursing care, treatments, and medication administration

• Supervising and supporting CNAs during daily care activities

• Monitoring resident conditions and reporting changes appropriately

• Assisting with admissions, transfers, and discharges

• Completing accurate and timely nursing documentation and incident reports

• Maintaining infection control, safety, and regulatory compliance standards

• Supporting a respectful, positive, and team-oriented environment for residents, families, and staff

• Following facility policies, nursing standards, and individualized care plans to ensure quality resident care

Requirements:

• Current, active Massachusetts LPN license

• Previous experience in long-term care, rehabilitation, or skilled nursing preferred

• Strong communication and teamwork skills

• Basic computer and documentation skills

• Ability to prioritize and make sound clinical decisions in a fast-paced environment

• Ability to communicate effectively with residents, families, physicians, and staff
