Location: Tarlani Healthcare Job Type: Full-Time/Part-Time Per Diem; Non-Exempt Pay: $50.00 – $140.00 per visit County Coverage: LA County, Orange County, San Diego County, Riverside County, San Bernardino County, Kern County, Ventura County Job Description The RN travels to a patient's home to provide direct care services, maintain patient independence, and ensure continuity of care. Responsibilities include administering at-home IVs, changing dressings, cleaning wounds, performing in‑depth assessments, overseeing treatment plans, and updating physicians and the agency on patient status. Documentation and communication with the agency are conducted through Synergy Software. Availability Patients require care 7 days a week. A flexible schedule is requested: at least two–three days per week (or ten productivity units), one weekend per month, and two holidays per year (one winter and one summer). Responsibilities Provide nurse visits to home‑health patients (home visits). Perform initial, comprehensive assessments and document results; upload to agency portal. Obtain and document physician orders. Initiate, re‑evaluate, and revise the plan of care in consultation with the Clinical Team Manager. Coordinate care with the Clinical Team Manager, physicians, and other home‑care team members; inform them of significant changes in patient condition and needs. Perform re‑certifications, resumption of care, transfers, and discharges as requested; complete and submit all related documentation. Assess the need for additional team members (PT, OT, ST, MSW, HHA). Perform Home Health Aide supervisory visits per regulatory requirements. Demonstrate knowledge of regulatory requirements. Requirements Current California RN license (required). BLS from the American Heart Association and/or the American Red Cross (required). NSO Nursing License Insurance (required). IV/Phlebotomy Certified (required). Comprehensive knowledge of OASIS documentation (preferred). Visiting RN for Home Health: 1 year experience (required). Nursing: 1 year experience (required). Qualifications Registered nurse with current licensure to practice in California. Graduate of National League for Nursing accredited school of nursing. Current CPR certification. Licensed driver with an insured vehicle in good working order. Minimum 2 years experience, at least one in public health or home‑care nursing (preferred). Self‑directed with ability to work independently. Excellent observation, problem‑solving, verbal and written communication skills. Ability to organize and prioritize workload; able to supervise home‑health aides. Physically capable of prolonged walking, standing, lifting, positioning, and transferring patients and supplies; adequate vision and hearing.
