Primary Care Physician

Role Overview The Primary Care Physician (PCP) delivers comprehensive, outcome-based care to a panel of up to 450 patients, focusing on acute and chronic illnesses in older adults. They act as health coaches, guiding patients toward short and long-term health goals through personalized care plans and behavior change strategies. Key Responsibilities Provide independent care, including assessment, diagnosis, treatment, management, and health promotion. Lead care teams comprising medical assistants, care facilitators, and coordinators, and collaborate with specialists and hospitalists for comprehensive patient management. Ensure compliance with clinical guidelines, quality standards, and organizational policies. Engage in patient-centered practices, ensuring accessibility, building trust, and promoting high-quality outcomes. Participate in risk and quality management, clinical meetings, and contribute to a positive center culture. Supervise and collaborate with Advanced Practice Practitioners, and assist in center operations and recruitment efforts. Qualifications & Skills MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar; board eligibility required. Current licensing and DEA registration; BLS certification within 90 days. Strong communication, leadership, and evidence-based medicine skills. Ability to build trust, influence outcomes, and adapt to flexible schedules. Proficiency in Microsoft Office and willingness to travel locally and regionally as needed.
