Admissions/Team Nurse

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families. The Admissions/Team Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager. Schedule: Monday-Friday; 8:30am - 5:00pm Location: Marin County with additional coverage to Sonoma County as needed By the Bay Health follows all CDPH vaccine requirements for healthcare personnel. Essential Duties & Responsibilities include (but are not limited to): Make in-person nursing visits to patients as assigned, or as indicated when case managing. Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals. Provide effective management of symptoms, including prompt escalation to providers or others as indicated. Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services. Make telephone contact and/or conduct assessment visits with potential hospice patients to provide information on agency services and assess patient status. Education/Experience: One year experience as a professional nurse in the past three years or Bachelor’s degree in nursing from an accredited School of Nursing. Experience in acute care/ home health and/or hospice setting preferred. Certificates, Licenses, Registrations: Current license as a registered professional nurse in the State of California. Public Health Nursing certification highly desirable. HPNA certification preferred.
