COMMUNITY HEALTH NURSE

Community Health NurseThis is a sensitive and responsible position that assists with the provision of direct health care delivery services to adults and children for the Florida Department of Health in St. Johns County (FDOH-SJC). This position is responsible for nursing duties in the areas of sexually transmitted infections, family planning and the provision of contraception, HIV testing and prevention (PrEP), pediatric and adult immunizations, tuberculosis testing and treatment and HIV care.Responsibilities include but are not limited to:Obtains and reviews prior medical records, reports and lab results for patientsObtains health historiesPerforms phlebotomy as ordered by the physician, advanced practice registered nurse (APRN) or physician's assistant (PA) or under nursing protocolPerforms nursing assessments and evaluations as appropriateFollows the Title X Family Planning Clinical Manual regarding the provision of non-directive counseling and support of patients making decisions related to contraception or basic fertility servicesAdministers medication and gives injections and immunizations, under the direction of the physician, APRN or PA or under nursing protocol as appropriateAssists with exams and proceduresReviews and documents immunization historiesEducates clients on health and disease processesProvides treatment and education for sexually transmitted infections when appropriateDocuments clearly, accurately and timelyThis position provides support to the communicable disease program. Responsibilities include but are not limited to:Screens, evaluates and assists with treatment for tuberculosis (TB)Provides nurse case management support to patients undergoing treatment for active or latent TBProvides rabies post-exposure prophylaxisAssists Epidemiology and/or Environmental Public Health with the investigation and follow-up of persons diagnosed with a reportable communicable disease by: Traveling to locate, interviewing and/or treating clients in the home, at a physician's office or in a hospitalCompleting follow-up and documentation as necessary for reportsPerforms other duties as assignedRequired knowledge, skills, and abilities:Knowledge of human anatomy and physiologySkilled in performing nursing/clinical processes and assessmentsAbility to communicate effectivelyAbility to plan and organize work assignmentsAbility to utilize an electronic health record and Microsoft ProductsAbility to problem solve and work in a team environmentQualifications:Licensure as Registered Nurse 464 F.SValid Florida Driver's LicenseThe benefits of working for the State of Florida include annual and sick leave benefits, paid holidays, state group insurance coverage options, retirement plan options, flexible spending accounts, tuition waivers, and more.
