Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment focused on healing and wellness. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. This includes the provision of nursing rounds, assessments, treatments, facilitating admissions, transfers, and discharges. Patient and family-centered education and care coordination are also essential. Key responsibilities include: - Performing and documenting a comprehensive assessment in accordance with RN scope of practice and policies/procedures. - Considering the patient’s culture, age, and developmental level. - Identifying patient risk factors and documenting presenting complaints. - Collecting objective and subjective data relevant to patient care. - Developing a problem list and prioritizing patient needs based on input from the healthcare team. - Fully implementing patient care therapies and procedures within established policies and guidelines. - Communicating procedures and treatments to patients and families to alleviate stress and discomfort. - Recognizing and responding to changes in patient condition appropriately. - Conducting ongoing assessments and documenting responses to procedures and therapies. - Delegating tasks appropriately and overseeing team members in patient care activities. - Monitoring the accuracy of delegated patient care provided by team members. - Identifying and addressing educational needs of patients related to prioritized problems. - Facilitating timely patient discharge processes and coordinating with Case Management. - Actively participating in unit-based activities that support organizational values. - Promoting a positive group dynamic and healing environment. - Ensuring nursing excellence in accordance with accepted standards and evidence-based practices. - Participating in quality improvement activities and core measures such as patient satisfaction and monitoring of documentation. - Utilizing a holistic perspective regarding patient care.
