We’re inviting an experienced and mission-driven Advanced Practice Provider (NP or PA) to join our Integrated Care at Home team. Qualifications Required: Current licensure as a Nurse Practitioner or Physician Assistant in the State of Michigan Practice independently with strong assessment, diagnostic, and care-planning skills Excellent communication & relationship-building abilities with patients, families, and teams Valid driver’s license with ability to travel throughout Berrien, Cass, and Van Buren counties Preferred: Experience providing care in home-based, community, or adult foster care settings Background in primary care, geriatrics, chronic disease management, or integrated care Ready to Practice Care with Purpose?
