Nurse Practitioner

Conviva Senior Primary Care is seeking a Nurse Practitioner to provide comprehensive primary care to adults 65+. In this role, you’ll deliver preventive, chronic, and acute care, emphasizing wellness, education, and long-term patient relationships. You will collaborate closely with an interdisciplinary team to coordinate value-based, patient-centered care and support seniors’ independence and quality of life.
