Job Details With a commitment to patient- and family-centered care, the professional nurse uses the nursing process in ways that value the uniqueness of each individual and address the physical, psychological, emotional, and social needs of the diversity of patients served by Jefferson Health Ambulatory Clinics. As an integral member of the ambulatory care clinical team, the nurse effectively collaborates with providers and support staff and provides clinical nursing interventions. The nurse communicates effectively and respectfully with patients to address individualized needs and priorities of patients that are consistent with their cultural beliefs and values. Must maintain a current nursing license in the state of practice. Job Duties Integrates Evidence-Based Practice, clinical expertise, patient, family/designated care partner preferences and cultural beliefs for delivery of optimal healthcare. Collaborates and communicates with clinic care providers regarding patient care and management; supports efficient and timely communication between primary care providers and specialists. Supports implementation of clinical care pathways for common care conditions and participates actively in quality improvement opportunities. Utilizes evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communication. Provides education to patients, their families/designated care partners for preventive care, chronic disease management, medication management, and management of common illnesses treated in the primary care setting. Documents all interactions and clinical care in EMR according to organizational standards and in compliance with regulations. Performs care coordination for patients such as abnormal results, triage, RN verification of vaccinations and other injections, and management of in-baskets (prior authorizations and forms). Oversees clinical care essential to assessment, promotion, maintenance and restoration of patients’ health and wellness. Minimizes risk of harm by following established nursing practices and protocols. Uses information technology to communicate, manage knowledge, and mitigate errors. Uses data to monitor care outcomes and support ethical decision making; applies improvement methods to design and test changes to continuously improve quality and safety. Functions effectively within ambulatory care inter‑professional teams, fostering open communication, mutual respect, and shared decision-making. Recognizes patients and their family/designated care partners in providing safe, compassionate and coordinated care based on respect for patients’ preferences, priorities, needs, and cultural beliefs. Creates an inclusive environment and demonstrates problem‑solving and conflict‑resolution skills. Demonstrates commitment to learning and seeks opportunities for professional growth. Actively participates in team huddles/assemblies to provide announcements and clinical operations updates. Demonstrates willingness to give and receive feedback and applies changes to practice. Minimum Qualifications Specialized Diploma Graduate of an accredited school of nursing. Knowledge of patient‑and-family‑centered practice and how to convey respect while collaborating. Knowledge of cultural awareness and ability to connect with diverse backgrounds. Understanding of effective communication concepts and techniques. Professionalism: honesty, confidentiality, integrity, attention to detail, time management, HIPAA compliance, ethical behavior per ANA standards, willingness to precept nursing students. Patient assessment and care knowledge of nursing and medical terminology; ability to assess, triage appropriately. Education knowledge of child and adult techniques, health literacy; ability to elicit learning priorities. Medication administration knowledge of pharmacology related to primary care, immunizations, and resources. Assessment and diagnosis knowledge of tools and techniques for patient assessment and nursing diagnosis. Health information documentation knowledge, ability to record required data in EMR; ability to complete EMR training and stay current. BLS (Basic Life Support) certification from AHA. RN license (and PA state of Pennsylvania) required. Preferred Qualifications Bachelor’s Degree BSN or completion of BSN or advanced degree within 3 years of hire. One year experience as a nurse in an ambulatory setting. Additional Qualifications Encouraged to obtain ambulatory care nursing certification. Expected participation in or support of quality, evidence‑based practice and research of nursing governance initiatives. Physical Demands Lift and carry 40‑50 lbs; push/pull patients on bed, stretcher (requires 29 lbs push force), lateral transfers up to 50 lbs of patient weight. Frequent to continuous standing/walking. Transporters may walk 8‑10 miles per shift. May be required to kneel, bend, squat and perform CPR.
