Job Opportunity: Nurse Practitioner
Our organization is in search of a dedicated Nurse Practitioner who is passionate about delivering primary and preventive healthcare to seniors participating in our PACE program. This role is integral to our interdisciplinary team, where you will:
- Conduct comprehensive assessments of patients.
- Diagnose and manage both acute and chronic health conditions.
- Prescribe necessary medications.
- Create personalized care plans tailored to individual needs.
- Work collaboratively with physicians, nurses, social workers, therapists, and caregivers to promote safe and independent living for our seniors.
The ideal candidate will be driven by a strong mission, uphold the values of dignity and respect, and excel in a collaborative environment that emphasizes holistic, person-centered care for the elderly.
