Registered Nurse Home Care
Johns Hopkins Medicine
Provides, coordinates, teaches, AND supervises nursing care for a group of patients in the setting of the patients' home. Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the nurse provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Case Manager ensures adequate service provision, including scheduling of visits, based on patients identified needs and priorities for care. Complies with all Home Health Services policies and procedures.
Here's what some of our Home Care Team Members say about their positions in Johns Hopkins Home Care Services:
"Having a position in homecare allows me to be the nurse that I have always wanted to be. The autonomy, along with being able to take enough time with my patients to ensure positive outcomes is very rewarding. My work/life balance is amazing with homecare because I am able to have a flexible schedule to meet the needs of my patients and my family." - Jessica H., RN Home Care
“What I find incredibly rewarding about being a home care nurse is the autonomy to support patients in their own homes by providing personalized care, advocating for their needs to enhance their quality of life, and building close relationships with not only your patients but their families as well. Each day can present new challenges and learning experiences that reinforce the importance of compassion and dedication in nursing, all creating a profound sense of fulfillment.” - Teresa H., RN Home Care
