RN Case Manager

POSITION SUMMARY

Under the direction of the Lead Nurse Case Manager, the Registered Nurse Case Manager will coordinate patient care services using the framework of the GMCH Mission Statement and the philosophy of pediatric palliative care. This is an hourly, part-time position scheduled for 24 hours per week. This role is fully onsite, with flexibility to adjust workdays based on department needs. Remote work is not available for this position.

ESSENTIAL FUNCTIONS OF THE JOB

Clinical / Technical At the direction of the patient’s primary/specialty care physician, the GMCH Medical Director, the Director of Clinical Programs, and the Lead Nurse Case Manager, coordinate palliative care plans for pediatric patients admitted for respite, transitional, and end-of-life care. Prior to a patient's admission, conduct thorough review of care with primary caregiver including, but not limited to medication administration, dietary regimen, respiratory support, bladder/bowel regimen, rescue protocols, durable medical equipment usage, and any additional treatments. Transcribe information with accuracy. Ensure required documentation is received and reviewed in a timely manner. Verify consents are on file and current. Obtain physician’s signature on admission orders and supplemental orders as needed. Follow up with clinics as needed. Calculate medications with 100% accuracy. In preparation of admission, enter required information into electronic medical record system (e.g., resources, care team, payers, demographics, etc.) and create medication admission record (MAR) by entering medications per signing provider with 100% accuracy. Attend weekly interdisciplinary team (IDT) meetings and take notes on current, discharged, and upcoming patient stays. In coordination with the IDT, develop, evaluate, and update the plan of care for each patient. Note patient status, concerns, follow-up, and plan of care. Enter notes into electronic medical record weekly. Routinely track follow-up tasks and check‑in with IDT members assigned specific tasks. Establish and maintain working relationships with the patient’s care team (e.g., managing physician, home health nurse, Regional Centers, social worker, etc.). Demonstrate the ability to recognize potential need for in‑services for bedside staff based upon assessment of intake orders. Communicate needs to Director of Clinical Programs and Assistant Director of Clinical Programs. Maintain current clinical knowledge of pediatric palliative care practices through ongoing education and attendance of in‑service training sessions. Support bedside nursing staff with administrative tasks, as necessary. Schedule respite admissions, update patient calendars, and communicate changes to appropriate team members. Document all communication with patients, primary caregivers, and care team in the medical record system. Support Lead RN Case Manager with insurance authorization (occasionally), data management, and referral tracking as needed. Performs other duties as requested. Patient/Family Interaction Meet with patients and families for tours and to facilitate preadmission paperwork. Facilitate family communication via phone calls and email. Assess and coordinate status of referrals, med changes, scheduling, updates, and a variety of other issues. Respond to case management inquiries in a timely manner. Attend family meetings to check in with primary caregivers as appropriate. Prior to each respite admission, review patient care needs and provide courtesy confirmation to the caregiver. In a timely manner, document all communication in the electronic medical record. Team Communication Prepare meeting materials as needed and actively participate in clinical meetings and scheduled Interdisciplinary Team meetings. Document discussion in electronic medical record. Maintain communication with patients, family, and hospital staff when a patient is transferred and/or hospitalized. Maintain patient confidence and protect GMCH operations by keeping all patient information confidential under HIPAA regulations. Coordinate with psychosocial team (i.e. Social Work, Child Life) to connect families to resources to help meet care needs. Collaborate with GMCH Dietary Team to address nutritional needs of each patient. Coordinate with on‑site medical interpreter and utilize interpretation services as needed. Obtain supplemental orders from on‑call provider as needed. Send IDT notifications as needed. Expertly navigate Microsoft Office Suite including Teams, Outlook, and Excel. Additional Participate in the development of new GMCH policies and procedures. Perform tasks in a culturally sensitive manner without regard to any protected category, as defined by federal, state, or local law. Utilize GMCH resources in a cost‑effective manner. Maintain a clean and organized work area. Effectively cope with personal, psychosocial, and bereavement concerns related to job duties. Maintain professional boundaries within the scope of practice. Adhere to all state regulations regarding professional licenses. Report to work on time for your scheduled hours, ready for work. Understand and adhere to GMCH compliance standards as they appear in the Staff Handbook, policies and procedures, and memos.

EDUCATION AND EXPERIENCE

Current California Registered Nurse license without limitations required. Current BLS/CPR certification. Two years of pediatric and/or end‑of‑life care experience preferred. Ability to communicate accurately and concisely using both written and verbal skills. Ability to work independently to manage a caseload of patients. Ability to access, input, and retrieve information from a computer. Knowledge of office procedures and office machines. Demonstrate the basic skills and core competencies of a Registered Nurse, including managing multiple, competing, and varied tasks, time management, listening skills, problem‑solving (clinical and interpersonal), decision making (clinical and interpersonal), delegation of duties and successfully managing workflow. Possess fluency in English. Possess fluency in Spanish or other languages (preferred). Possess a valid California Driver’s License with a good driving record.
