Our innovative Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping people heal at home, with dignity and purpose.We are proud to have ranked in Boston Globe's Best Places to Work in Massachusetts and USA Today's Best Places to Work in the United States since 2023!We will consider RNs and LPNs for this role!Position Details:Main Office location: Springfield, VirginiaService Area: Reston, Centreville and Surrounding AreasJob Type: Full TimeSchedule: Monday - Friday; Weekend Rotation (not on-call)Responsibilities:Patient Care:Assesses and evaluates patient's status by:Writing and initiating plan of care.Regularly re-evaluating patient and family/caregiver needs.Participating in revising the plan of care as necessary.Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.Provides and maintains a safe environment for the patient.Attends interdisciplinary group meetings.Additional Duties:Participates in on-call duties.Ensures that arrangements for equipment and other necessary items and services are available.Supervises ancillary personnel and delegates responsibilities when required.Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.Fulfills the obligation of requested and/or accepted case assignments.Actively participates in quality assessment performance improvement teams and activities.Qualifications:2+ years' nursing experience, preferably in geriatrics is desirable.Degree in Nursing from an accredited institution and an active nursing license in Virginia.Proficient with electronic medical records.Excellent bedside manner and communication skills.Working knowledge of health care laws and regulations.Maintain current BLS CPR certification.Possess a valid driver's license and own a reliable, insured vehicle.Benefits & Compensation:Comprehensive benefits package including 401(k) match, PPO Plan, Dental, Vision, Life Insurance, Short-Term Disability, and Flexible Spending Account.Flexible schedule to promote work-life balance.Opportunities for leadership development and professional growth.Consistent day-shift hours and work schedule.Pathways Healthcare is committed to providing exceptional care to our patients and fostering a positive work environment for our team members.
