Our therapy department partners closely with nursing, physicians, and case management to create individualized treatment plans that help residents achieve their highest level of independence and quality of life.We pride ourselves on fostering a positive workplace culture where teamwork, compassion, and clinical excellence are valued and recognized.ResponsibilitiesEvaluate residents and develop individualized occupational therapy treatment plansProvide skilled occupational therapy interventions in accordance with physician orders and established care plansHelp patients improve performance in activities of daily living (ADLs), self-care tasks, mobility, cognition, and functional independenceCollaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and families to coordinate careComplete timely and accurate documentation in compliance with regulatory and facility requirementsParticipate in interdisciplinary team meetings, care conferences, and discharge planningEducate patients, families, and caregivers regarding therapeutic techniques and home programsMaintain compliance with all federal, state, and professional standards of practiceQualificationsCurrent California Occupational Therapist (OT) license in good standingGraduate of an accredited Occupational Therapy programSkilled nursing, post-acute, rehabilitation, or geriatric experience preferredNew graduates with strong clinical rotations are encouraged to applyExperience with Casamba, Net Health, PointClickCare, or similar EMR systems is a plusStrong communication, organizational, and teamwork skillsPassion for helping patients achieve their highest level of function and independenceIdeal CandidateYou are someone who:Brings positivity and compassion to every patient interactionThrives in a team-oriented environmentValues clinical excellence and patient-centered careIs committed to helping patients achieve meaningful outcomesEnjoys working with the geriatric and post-acute populationJoin Our Rehab Team Today!At Santa Rosa Post Acute, you'll be part of a team that truly makes a difference.
