Geriatric Nurse Practitioner (NP)

Job Summary:

Carease Health is seeking a highly skilled and compassionate Geriatric Nurse Practitioner (NP) to join our team in Irondequoit, New York. As a key member of our healthcare organization, you will play a vital role in providing high-quality, patient-centered care to our geriatric patients. Our comprehensive preventative care program incorporates Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to prioritize proactive measures for enhanced physical health and well-being.

Responsibilities:

• Provide primary and specialty care to geriatric patients, including physical exams, diagnostic testing, and treatment planning

• Conduct patient assessments, develop personalized care plans, and implement evidence-based treatments

• Collaborate with healthcare teams, including physicians, social workers, and other healthcare professionals to ensure comprehensive care

• Utilize RPM, CCM, and CoCM to monitor patient health and detect abnormalities swiftly

• Develop and maintain strong relationships with patients, families, and caregivers to ensure exceptional patient satisfaction

• Participate in ongoing quality improvement initiatives to enhance patient outcomes and reduce hospitalizations
