SOC NurseThe SOC Nurse is responsible for providing comprehensive nursing assessments and initiating home care services for patients transitioning into care. The SOC Nurse conducts Start of Care visits, develops and implements individualized plans of care, coordinates multidisciplinary services, and ensures compliance with regulatory, agency, and payer requirements. The role requires strong clinical judgment, care coordination, documentation, and communication skills to support safe and effective patient outcomes.
