SummaryCase Manager is responsible for directing, coordinating and monitoring the treatment,utilization and general care of children within the residential program. Directs the treatmentteam in treatment planning. Facilitates intake and discharge planning with the ECH Screener,insurance companies, families, court representatives, and CD workers.Essential Duties and Responsibilities include the following. Other duties may be assigned.Responsible for being trauma aware, sensitive and responsive to youth, families andcolleagues through the completion of new hire and annual trauma informededucation/training.Completes admission process with the child, referral source, family/legal guardian on theday of placementCompletes administrative functions associated with placement. Receives consents,collects documents and receives releasesCompletes Admission Assessment, Initial Service Plan and visitation and phoneprivilegesConvenes and directs micro meetings and oversees service plan implementationMonitors and evaluates treatment team, child, family and legal guardians' progresstowards desired outcomesSchedules and directs Service Plan ReviewsCommunicates relevant information to all concerned parties between reviewsResponsible for discharge planningRepresents Agency and service team at judicial hearings, IEP meetings, psychiatricappointments, and other administrative meetingsPrimary contact for CD, DMH, DJO, and other managed care entitiesPrimary contact for families and visiting resources. Develops and oversees visitationplansOn Duty expectations—Due to the significant level of mental health issues experiencedby our clients, a clinician is required to be available 24 hours a day to make therapeuticdecisions to improve mental health status. Additionally, clinicians determine the need forpsychiatric hospitalization when warrantedFunctions as a team leaderSafe Crisis Management (SCM) competencyResponsible for school registration, parent/teacher conferencesCoordinates educational services with on-ground school staff, Normandy School staff,and/or Special School District schoolComplete progress notes in a timely mannerAttend and complete trainings as established by ECHOther duties as assigned by supervisorRequirements:A master’s degree in counseling or social work (M.A. or MSW) is required for this position.This person should have a minimum of 900 hours clinical practical experience #J-18808-Ljbffr Every Child
