Occupational Therapist, Acute Care — Oakland, California. Responsibilities
- Assess residents to identify occupational therapy needs and develop individualized treatment plans to improve ADLs, mobility, and functional independence.
- Deliver therapy sessions focused on upper-extremity function, cognitive skills, and adaptive equipment training; monitor progress toward goals.
- Educate patients and families on therapy plans, home exercise programs, and safe activity participation; coordinate discharge planning as appropriate.
- Complete timely documentation of evaluations, progress notes, and discharge summaries in the electronic medical record; ensure compliance with regulatory and facility policies.
- Collaborate with the interdisciplinary care team (nursing, therapy, and physicians) to ensure a coordinated plan of care and safety for patients.
- Participate in care rounds and contribute to quality improvement efforts as applicable.

Requirements
- Ability to meet credentialing and licensure requirements for a travel OT role (as applicable)
- Strong communication and teamwork skills to collaborate with diverse care teams
- Proficiency in OT evaluation tools and documentation within an electronic health record system
- Flexibility to work in a skilled nursing facility environment with an acute care-informed approach
- Availability to work a 5x8 day shift with guaranteed hours
