Job Summary (List Format): Nurse Practitioner
- Provide primary and family evidence-based care, including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
- Collaborate daily with a clinical care team to drive better outcomes for a panel of patients.
- Perform administrative duties such as clinic opening/closing, inventory management, patient follow-up, insurance verification, payment collection, and maintaining a clean environment.
- Handle patient appointments and routine examinations to ensure positive health outcomes.
- Conduct diagnostic tests and coordinate with other healthcare professionals for comprehensive care planning.
- Educate and encourage patients to participate in their own healthcare and well-being, including health screenings, behavioral health, wellness, and chronic condition management.
- Work three out of every six weekends, with possible holiday rotations; may require on-call participation and occasional travel for float work.
- Complete candidate questionnaire as part of the application process.
- Ensure all licensure, BLS (ARC or AHA), and possible DEA registration requirements are met.
- Practice autonomously with minimal supervision while being part of a team-oriented environment.
- Maintain outstanding organizational, communication, and multitasking skills.
- Utilize electronic medical systems (EMS) for patient documentation and care coordination.
- Report directly to the practice manager.
Minimum Requirements:
- At least 1 year of recent experience as a Nurse Practitioner in primary care, family medicine, or urgent care.
- Master's Degree in Family Nurse Practitioner program, with current national board certification and state license.
- BLS certification (ARC or AHA).
- Working knowledge of quality measurement in chronic disease management.
- Willingness to obtain multi-state licensures if needed.
- Ability to work onsite and provide in-person, patient-facing care.
