RN - Coordination of Care Specialist

This role requires collaboration with physicians, healthcare professionals, and support staff to meet and exceed quality standards and regulatory requirements. The ideal candidate is adaptable, open-minded, and committed to improving patient outcomes through effective care coordination.
- Current Registered Nurse (RN) license required.
- Minimum of two years in an acute care environment.
- Preferred experience in Medical/Surgical and/or ICU settings.
- Case management experience is a plus.
- Demonstrated critical thinking and above-average clinical competence.
- Excellent interpersonal, negotiation, and communication skills across diverse social, economic, and cultural backgrounds.
- Self-motivated with strong written, telephonic, and electronic communication abilities.
- Ability to discuss clinical, socio-economic, and financial patient issues effectively with physicians and patient representatives.
- Strong organizational and time management skills to prioritize multiple tasks efficiently.
- Proficiency with Microsoft Office and electronic medical record systems.
- Ability to foster teamwork, respect, and positive change within a multidisciplinary healthcare environment.
- Physical ability to perform light lifting and walking as needed within the facility.
