Assesses spiritual needs of patients and provides appropriate interventions considering growth development needs and age group. Receives referrals from the EPIC EMR work queue and follows up within 24 hours. Responds to incoming calls and conducts necessary follow-up on inquiries. Processes, documents, and completes referrals within the Epic EMR work queue in accordance with established workflows and departmental standards. Develops spiritual care plans for patients by identifying objectives, interventions, and desired outcomes. Identifies and shares relevant community resources aligned with the caller's specific needs and local area. Utilizes knowledge of different levels of growth, development, and skills for various ages and demographics of patient care. Available for various shifts as scheduled by the Spiritual Care Manager. Performs other duties as assigned.
