Key Responsibilities

• Conduct comprehensive evaluations of patients in their home environments, developing individualized treatment plans that address specific needs and goals.
• Administer physical therapy treatments for patients recovering from orthopedic injuries, surgeries, or chronic conditions, utilizing techniques such as exercise therapy, manual therapy, and functional activity training.
• Design and implement tailored exercise programs aimed at improving strength, flexibility, and mobility, ensuring each patient understands the purpose and benefits of their regimen.
• Continuously monitor and document patient progress, modifying treatment plans as necessary to optimize recovery outcomes.
• Educate patients and their caregivers about the rehabilitation process, assistive devices, and self-care techniques to encourage independence and long-term success.
• Participate in clinical meetings and case discussions to enhance treatment effectiveness and share insights with colleagues.
• Engage regularly in professional development activities such as attending workshops, participating in research, and networking with other healthcare professionals to stay abreast of advancements in physical therapy.

Patient Population / Client Focus

Your work will focus on a diverse population of patients, ranging from post-operative orthopedic surgery patients to those dealing with chronic pain or mobility limitations. By providing care in their homes, you will gain unique insight into their living environments, allowing you to better understand and address their individual challenges and motivations.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• A valid State License in Physical Therapy to practice within Virginia.
• Excellent communication skills, both written and verbal, allowing you to effectively engage with patients and collaborate with healthcare teams.
• Proficient in using electronic health record (EHR) systems, word processing, and other relevant digital tools.
• Demonstrated flexibility and adaptability to meet each patient's unique needs.

Preferred Experience

Prior experience in home health physical therapy is preferred but not mandatory. We welcome recent graduates who bring enthusiasm and a fresh perspective on patient care.

Compensation and Benefits
